Hello. Regarding the fifth treasure, it's most likely a visual bug and that part is already counted as completed (it just shows 0 due to the bug). If you have 24 key fragments and see the fifth treasure along with something else that hasn’t been completed yet, then 99.99% of the time, it means you still need to do that other task. Also, remember that the third location is the Sanctuary, and in order to collect the key fragment from the Sanctuary, you need to have completed the quest for that loc...
